[ARCHIVED] Emergency Preparedness Survey

A group of Public Health individuals are involved in leadership training with the Great Plains Public Health Leadership Institute. As a team project, the group is currently looking at emergency preparedness of individuals and communities.

The purpose of the survey is to better understand the experience of citizens directly impacted by severe weather events occurring in Nebraska within the past year; including during and after the storms. In an effort to identify any gaps in plans that are currently in place, we are asking you to complete a survey that involves your experience with recent tornados that have occurred in your area.

The information collected will be used in the study described above. You will not be asked to identify yourself and your participation will remain confidential.
